7/29/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, JaCaeri made noises, blurted out, and talking out excessively for the purpose of seeking attention from adult figures/peers. JaCaeri began to display playful, silly behaviors as evidenced by making inappropriate noises and looking for attention. 2. Behavior: JaCaeri presented symptoms of withdrawn at times within the session as he was playing with his phone and touching other items within the home as he needed multiple redirection from QMHP-C as well as his mother. 3. Behavior: During the second portion of the session, JaCaeri was displaying hyperactive behaviors such as him leaving the room when discussing his behaviors with his mother. 1.Intervention:. QMHP-C confronted negative behaviors and explored the purpose of JaCaeriâs behaviors. QMHP-C provided JaCaeri more positive alternatives to behavior. QMHP-C offered positive feedback for more appropriate behaviors exhibited. QMHP-C reiterated the importance of attempting to work more independently and only seeking assistance when necessary. QMHP-C explained how behaviors are counter-productive to achieving personal and treatment goals, and offered JaCaeri more positive productive options to behaviors. 2. Intervention: QMHP-C build therapeutic trust and rapport with consistent eye contact, active listening, unconditional positive regard, and warm acceptance. QMHP-C utilized reward system/verbal praise to reinforce JaCaeriâs initiation of verbalization. 3. Intervention: QMHP-C utilized a therapeutic worksheet called âWhat is R-E-S-P-E-C-Tâ. JaCaeri completed the exercises from the worksheet and counselor went over it with him. QMHP-C talked to JaCaeri about learning that there is a right time to play and a wrong time to be playful. QMHP-C engaged JaCaeri in a game to assist him with patience, and positive communication skills. QMHP-C processed on anger control and coping skill such as journaling and building constructive actions to direct his anger/frustrations. 1.Response: JaCaeri seemed to be nonchalant towards intervention by walking away on several occasions. JaCaeri was not focused by looking around and focusing on his surroundings. 2. Response: JaCaeri was receptive after several occasions but struggled with interacting and understand himself and his actions developed. Jashim reports to QMHP-C on how he continues to do what he is supposed to be doing in the home but it is others that make him mad so when he is mad he is not going to do anything anyone tells him to do. 3. Response: Mother verbalized that he was tired of JaCaeri's actions and felt that he do things to agitate him JaCaeri interacted with counselor in a positive manner and was able to demonstrate positive communication.
